Nowadays, home care is growing rapidly. Each region takes a somewhat different approach to its delivery. However, the attributes considered to contribute to high quality, such as continuity of care, are not clearly described and are not determined. We sought to spot factors that were significant to experiencing continuity of care in home care. We interviewed home care customers and their caregivers, staff in the home care system and physicians whose patients utilize home care Warrington. During in-depth interviews with these key stakeholders, they illustrated the conditions that led to stability of care in home care. Service providers and case managers were also solicited about the types of customers who need a high level of care continuity.
Care that is qualified as running smoothly, that responds to customers’ needs and requires no special effort for customers to maintain, was noticed as having continuity. The attributes of care experienced as easing continuity could be grouped under 2 scope of care – managing care and direct service provision. Different stakeholders put emphasis on different attributes of care as most essential to continuity. Customers included steadiness of timing of service delivery while rarely mentioning care management issues. They emphasized the importance of consistent knowledge and skills in the workers and trusting relationships as significant to experiencing care continuity. The depiction of attributes of continuity of home care that appeared from this study is contrast to definitions found in the nursing, mental health and primary care literature.
